Study Also Finds Disparities in Adult Dental Visits by Race, Immigration

Visits to the dentist drop significantly after adults turn 80, finds a new study by researchers at NYU Rory Meyers College of Nursing and the University of Hawai‘i at Mānoa.

The study, published online in the journal Research on Aging, also highlights disparities in dental visits for U.S. adults by race and country of birth, with immigrants and racial and ethnic minorities less likely to access care.

Oral health is increasingly recognized as an essential part of healthy aging. It is closely related to overall health status and quality of life, and regular dental checkups can prevent oral diseases and maintain good oral health.

However, regularly seeing a dentist is a challenge for many Americans, especially older adults, racial and ethnic minorities, and immigrant populations. Older adults face barriers such as a lack of access to quality dental care, awareness of the importance of oral health, and dental insurance coverage. Medicare does not cover most dental care, and only 12 percent of Medicare beneficiaries report having at least some dental insurance from another source to help pay dental expenses. These roadblocks to dental care increase for racial and ethnic minorities and immigrants, who may experience racial discrimination and language barriers in healthcare settings.

“To promote oral health and close racial and ethnic gaps in oral health disparities, seeing a dentist regularly is critical,” said Wei Zhang, PhD, professor and chair of the Department of Sociology at the University of Hawai‘i at Mānoa and the study’s first author. “Failure to engage in preventive dental care may lead to serious consequences such as tooth decay, pain, tooth loss, and inflammation.”

In this study, the researchers examined how often people see a dentist as they age, focusing on U.S. adults 51 years and older, and explored variations by race and country of birth. While previous studies have looked at recent trends of dental care utilization among adults in the U.S., this study extends these efforts by using longitudinal data to focus on middle-aged and older adults across an extended period of time.

The researchers used data from the Health and Retirement Study (HRS), a longitudinal study from the University of Michigan that conducts interviews with a national sample of middle-aged and older adults. They analyzed rates of dental care utilization—measured by whether someone had seen a dentist in the past two years—for 20,488 study participants of different races and ethnicities, including 17,661 U.S.-born and 2,827 foreign-born individuals.

Seventy percent of adults had visited a dentist in the past two years, but this rate decreased significantly beginning around age 80. U.S.-born adults of all races and ethnicities were more likely to see a dentist (71 percent) than immigrants (62 percent). Interestingly, the gap in care between U.S.-born adults and immigrants shrunk as people aged, suggesting that age and acculturation may play a role in decreasing oral health disparities over time.

The researchers also found that White adults had higher rates of service utilization than Black and Hispanic adults, and while the rates of service utilization decreased with age for all groups, the rates of decline for Whites were slower than others.

“Our study went beyond prior research by confirming that racial and ethnic disparities were substantial and persistent as people became older, regardless of their birthplace and while adjusting for a wide range of factors. This finding is alarming as it indicates that some unmeasured factors beyond the scope of this study, such as oral health literacy, perception of need, barriers to access, and dissatisfaction with dental care, could play important roles in explaining the disparities in dental care as people age,” said Bei Wu, PhD, Dean's Professor in Global Health at NYU Rory Meyers College of Nursing and co-director of the NYU Aging Incubator, as well as the study’s senior author.

The study’s findings highlight the importance of identifying racial and ethnic barriers to dental care for aging adults and developing culturally competent programs to meet the dental needs of the increasingly diverse U.S. population.

In addition to Zhang and Wu, the study was co-authored by Yan Yan Wu, PhD, of the University of Hawai‘i at Mānoa.

The Pacific Dental Services®(PDS) Foundation is pleased to announce its partnership with the Key Free Clinic in Lakebay, Washington to provide donated dentistry to underserved patients in need. Joining forces with The Smile Generation®, on Saturday, July 27 the Foundation’s Mobile Dental Clinic (MDC) will be onsite at the clinic from 7:30 AM – 3:00 PM. Smile Generation-trusted dentists and volunteers will provide patients with comprehensive oral health care, including exams, x-rays, cleanings, extractions, fillings, and same-day crowns.  Jessica Ahn, DMD, a Smile Generation-trusted owner dentist of multiple dental practices throughout the state of Washington, will be on hand to serve patients on Saturday.

“I’m honored to serve the patients in Lakebay as part of our partnership with the Key Free Clinic,” said Dr. Ahn. “My goal is to make going to the dentist enjoyable for all. I look forward to meeting new patients this Saturday at the Key Free Clinic and helping them create healthier, happier smiles.” Opening its doors in 2013, the Key Free Clinic provides free medical care to people who cannot afford or reach health care. MGE: Management Experts Raises More Than $100,000 to Help Children Access Dental Care

More kids will have a reason to smile thanks to MGE: Management Experts, Inc. The practice management training organization held a high-energy auction benefitting America’s ToothFairy during their recent Owner’s Conference in Tampa, Florida. The conference, which is held each year to honor the accomplishments of MGE’s clients, was attended by more than 300 participants of their executive training program.

“Our mission is to help our clients achieve their goals and lasting success in practice,” MGE CEO, Luis Colon remarked after the event. “We could not be more proud of the generosity that flows out from their success. We are so pleased to be able to help America’s ToothFairy, a wonderful organization that has made a remarkable difference in the lives of kids in need of dental care and education.”

“We were so honored when MGE offered to hold this fundraiser on our behalf and absolutely blown-away by the results,” said America’s ToothFairy Executive Director, Jill Malmgren. “We are thrilled to be able to help even more kids in need of urgent dental care, and are excited to apply a portion of the funds raised to our new Smile Guardian Education Initiative, which will teach thousands of children how to prevent the suffering that comes from dental disease. Funds raised by the auction will be used to pay for the treatment of 30 children in need of urgent or restorative dental care through America’s ToothFairy’s In the Gap Program; distribute educational resource kits to nonprofit dental clinics, schools, and community organizations with the combined capacity to reach at least 10,000 children with oral health lessons; and award a grant to provide dental screenings and cleanings to a minimum of 2,000 children in need.

America’s ToothFairy Board of Directors member, Dr. Gordon Christensen held a seminar during the three-day conference and graciously posed for photos with hundreds of event attendees. “In all the years I have been involved with America’s ToothFairy, I have never seen people so motivated and inspired to make a difference,” he said. “We have been so energized by their generous spirit!”

Dentsply Sirona Supports TeamSmile to Make Children Smile

Since 2007, TeamSmile's mission has been to facilitate partnerships between dental professionals, professional sports teams, and dental organizations. They work together to provide dental treatment for children who otherwise would have no access to quality dental care and oral health education in their communities. Dentsply Sirona supports this great initiative. During an event in Tampa, 275 children were successfully treated to make them smile again...one child at a time.

On Friday, July 12, the TeamSmile truck and support team stopped in Tampa, FL. Thanks to this TeamSmile initiative supported by Dentsply Sirona, dental professionals successfully treated 275 children. Dr. Terri Dolan, Chief Clinical Officer at Dentsply Sirona, and Keith Clements, Vice President of US Distribution & Customer Support at Dentsply Sirona, handed over a check to John McCarthy, Executive Director of TeamSmile at the Tampa Bay Rays Stadium in Tampa.

In conjunction with the company’s financial donation, Dentsply Sirona donated consumables and equipment, while local sales associates from Dentsply Sirona volunteered at the event. The associates spent time interacting with the children, staying with them through their dental treatments and ensuring that it was a positive and stress-free experience. They also accompanied the children to the fun activities, including a pizza lunch, playing baseball with trainers from the Tampa Bay Rays, face painting, balloons, running around the baseball field, or dancing to the music and enjoying the DJ. TeamSmile wants to ensure kids have a lot of fun while receiving dental care – sometimes for the first time.

Dentsply Sirona began supporting TeamSmile in 2017 at a local Charlotte, NC, event. Based on this positive experience, Dentsply Sirona expanded its support to become a national partner. Dr. Terri Dolan said, “I'm so impressed with TeamSmile’s leadership, mission, and commitment to improving the oral health of at-risk children. It is a great joy and matter of the heart to become a national partner of TeamSmile. The events bring healthy smiles and oral health to children and provide volunteer opportunities for our team members. They get to see first-hand how the TeamSmile, Dentsply Sirona, and community partnerships positively influence children's lives."

In 2007, Dr. Bill Busch, a Kansas City dentist, was inspired by an NBC Nightly News story about a young boy who died as the result of an untreated abscessed tooth due to the family having limited resources and lacking access to health care. Knowing that this tragedy could have easily been prevented, Dr. Bill Busch decided to take action and make a positive impact in his own community. TeamSmile hosted its first outreach program in 2007.

TeamSmile operates as a national non-profit organization. Every year, its program takes place in multiple cities and states around the US, supported by local professional or major college sports teams. In 2018, the organization conducted 29 events, providing more than $2 million worth of free dental care and oral health education to more than 5,400 children. In 2019, they are scheduling between 32-35 events with the goal of providing care for 6,000 children.

In partnership with The Smile Generation®, on Saturday, July 13, 23 Smile Generation-trusted dentists and volunteers provided $22,211 of donated dentistry to 24 patients.

Integration Enables Centralized Images in the Patient Chart Generated from All Planmeca Devices

Henry Schein One announced today the full integration of Dentrix® G7 and Dentrix Smart Image with the Planmeca Romexis software suite, which for the first time ever, allows images generated from any Planmeca device into the Dentrix patient chart, creating a seamless workflow for dental professionals.

Part of Dentrix G7, the latest upgrade to Dentrix practice management software, Dentrix Smart Image allows dental professionals to view various image types from several participating third party vendors in the Dentrix Patient Chart, helping to automate common imaging and practice management-related activities. By integrating Planmeca Romexis with Dentrix Smart Image, practitioners can access images generated from any Planmeca device, including Panoramic, 2D intraoral and extraoral imaging units, 3D systems and CAD/CAM, from a single place in Dentrix – the Patient Chart – which improves workflows for case diagnosis and acceptance.

In addition to automatically transferring images into the Patient Chart, Dentrix Smart Image automates workflows for Planmeca users by posting individual procedures to the ledger to facilitate payment, updating the patient’s record for accurate tracking of eligibility dates for new images, and adding an entry in the patient’s progress notes. With the upcoming release of Dentrix G7.3, Planmeca images will be automatically attached to insurance eclaims, as well.

Lancet Series on Oral Health, Co-Authored by NYU Dentistry’s Habib Benzian, Calls for “Radical Reform” of Oral Healthcare

A special Lancet Series on Oral Health, published today in The Lancet, presents an “urgent need for radical reform” of oral healthcare to prioritize prevention and integrate dentistry into primary care. The series is comprised of two papers, both co-authored by Habib Benzian, DDS, MScDPH, PhD, the associate director of global health and policy for NYU College of Dentistry’s World Health Organization (WHO) Collaborating Center for Quality-improvement, Evidence-based Dentistry—the only WHO Collaborating Center on oral health in the Americas.

Oral diseases, including tooth decay, gum disease, and oral cancers, are major global public health problems affecting half of the world’s population, but they have gone largely unnoticed.

“This immense disease burden is an important starting point to rethink the current oral healthcare approach, which leaves large parts of the world’s population without access to even basic dental care,” said Benzian, who is also an adjunct professor at NYU’s Colleges of Dentistry and Global Public Health. “By dedicating a special series to oral health, The Lancet is creating visibility and urgency for a long-overlooked public health crisis, and my co-authors and I offer solutions to improve global oral health and reduce inequalities.”

In high-income countries, oral healthcare is dominated by treatment—including high-tech, high-cost interventions—rather than tackling the underlying causes of disease and preventing it.  In low- and middle-income countries, however, dentistry is largely unavailable and unaffordable.

As part of prioritizing prevention, the Lancet Series also examines the need to reduce sugar, alcohol, and tobacco consumption to improve oral health and other chronic conditions.  Sugar, in particular, is recognized as the leading cause of tooth decay. The authors call for the adoption of a range of policy changes to protect and improve health, including taxing sugary drinks and stronger regulation on advertising targeting children.

The Lancet Series also explores the problematic rift among oral healthcare, mainstream healthcare, and health policy.  The series provides recommendations for system-wide reforms.

“Oral healthcare is isolated from mainstream healthcare. Thus we need a fundamentally different approach that integrates the two. A reformed oral healthcare system needs to incentivize prevention, embrace a wider team of health professionals, including primary care and mid-level providers, use evidence-based treatments, and respond to the diverse needs of local populations,” said Benzian. “The growing global momentum towards universal health coverage provides a unique opportunity to reform oral healthcare. Bold efforts in Brazil and Thailand have demonstrated that major reforms are possible and can benefit oral health. Countries like France have embraced the principles of universal health coverage and are now changing their systems to include oral healthcare in innovative ways.”

“Prevention is a key factor in reforming oral healthcare, and it is currently underutilized,” added Richard Niederman, DMD, chair of the Department of Epidemiology & Health Promotion at the NYU College of Dentistry and director of the WHO Collaborating Center. “Our studies of cavity prevention programs for U.S. children show that these programs prevent tooth decay, improve school attendance and performance, and are cost-effective and cost-saving.”

The Lancet Series on Oral Health was led by researchers at the University College London and brings together 13 academic experts from 10 countries. The Lancet will launch the new series on Wednesday, July 24 in London. Benzian will share key policy recommendations at the event, speaking alongside other authors, invited global health leaders, and The Lancet’s editor-in-chief, Richard Horton.

NYU Dentistry’s WHO Collaborating Center will continue the conversation on reforming oral healthcare and universal health coverage with a United Nations (UN) side event in collaboration with The Lancet and the World Economic Forum. The event will take place September 22 in New York City, preceding the UN High-Level Meeting on Universal Health Coverage on September 23 during the UN General Assembly high-level week.

UTHealth Teaches Students How to Make Same-Day Crowns

If you have a damaged tooth that needs to be capped with a crown in a hurry, you will want to be seen by a graduate of The University of Texas Health Science Center at Houston (UTHealth) School of Dentistry.

The school recently invested close to $1 million in its digital dentistry program to make sure all students earning a Doctor of Dental Surgery or DDS can make same-day crowns.

Traditionally, doctors make a mold, send it off to have a crown fabricated, and then ask the patient to come back in a week or two to have the crown installed.

Today’s digital dentists are using lasers to make impressions of damaged teeth and tiny mills to make the crowns themselves. There are no return visits, no molds of teeth, and no temporary crowns.

“Over the next five years, there will be a huge transition in dentistry in which more than 80% of all crowns will be designed and milled in-house,” said Michelle Thompson, DDS,  director of digital dentistry and assistant professor at UTHealth School of Dentistry.

Thompson, who completed her general practice residency at UTHealth, said, “We want to stay ahead of the curve when it comes to the latest technology.”

This is a win-win for the school and the dental community.

“We’re going to be able to attract more applicants with this technology. Also, our grads will be able to walk into a dental practice and perform these high-tech procedures,” said Stephen Laman, DDS, digital dentistry instructor, associate professor, and group practice director with the school.

In all, the school acquired 27 new laser scanners and five new mills. A six-week summer orientation session was scheduled for faculty members along with students.

In the training, a student works in front of an exam table with the upper torso of a mannequin. The student inserts a laser device into the mouth of the dummy and an exact image of the damaged tooth appears on a computer screen.

“This is the same three-dimensional technology that engineers use to design airplanes and cars. Light bounces off the damaged teeth. It’s then relayed to the computer, which in turn designs an exact replica,” said Laman, noting that this technology is formally called computer-aided design/computer-aided manufacturing or CAD/CAM.

Laman has been repairing damaged teeth for decades and said the CAD/CAM-generated crowns are more precise than those made with a mold of the damaged teeth. “We have been monitoring this technology for years and it’s at the point where equivalent or better results can be obtained,” Laman said.

Maria Loza, DMD, MS, who chairs the Department of Restorative Dentistry and Prosthodontics, is also training students.

“The move to digital dentistry began with digital photography, digital radiographs, and electronic health records. Now, we’re using this technology to make replacement teeth. In the future, I could see us also making bridges, implant crowns, and dentures with this technology,” said Loza, the Yun J. Ahn and Song Ahn, DDS Professor in Implant Dentistry at UTHealth.

Thompson said the school’s ultimate goal is for upper-level students to make same-day crowns in a School of Dentistry clinic.
