Class V Restorations with Cervical Former

A step-by-step application.

The new Cervical Former Kit is a matrix system that allows clinicians to easily obtain smooth and homogeneous class V fillings with an optimal esthetic result. The kit is composed of a long, bent holder for posterior necks, a short holder for anterior teeth, and 50 cervical matrices, available also in refill packs. Both holders are autoclavable.

The cervical matrices are made of transparent polyester, providing the following benefits:

  • Flexibility: The matrices, if bended along their vertical axis, can be easily introduced into the holder and will adapt perfectly to the neck of the tooth during the application.
  • Rigidity: The matrices are able to exercise the proper pressure on composite during polymerization, avoiding air bubbles, porosities, or composite waste due to overflow.
  • Adaptability: The matrices can be reshaped with usual scissors if needed.
  • Multifunctionality: The matrices protect the tooth from moisture and do not stick to composite materials.
  • Universality: The matrices can be used with all types of composites.
  • Profitability: There is an ideal relationship between finishing time and result.
  • All-in-one: The system includes two matrices in one, thanks to the different end shapes.

Clinical Case

The patient was a 51-year-old non-smoking woman with a good general health and close to perfect hygiene, who was unsatisfied with the esthetics of the resin composite restorations done on her front teeth 20 years prior (Figure 1).

The surface of the involved tooth was cleaned with a fluoride-free polishing paste and a brush. The old restorative material and new decay were removed, and the enamel surrounding the cavity was sloped in the width of at least 0.5 mm in order to get better marginal sealing. The access to the oral cavity was optimized using a check and lip retractor. Whenever possible, a rubber dam is recommended. The matrix was easily adjusted with scissors as needed, according to the required shape (Figure 2).

The tooth surface was etched, coated with an adhesive system, and polymerized according to recommended application protocols. A resin composite was then applied to the anterior tooth and in a small amount on the matrix itself. The matrix was then placed on the tooth surface by exerting a reduced pressure. In the case of deeper cavity preparations, it is of course recommended to apply the resin composite in more than one layer (Figure 3). The final ideal shape and surface was obtained using very fine diamond burs and silicone polishers (Polydentia Composite Polishers). The final result was a high-quality direct restoration that satisfied the patient, achieved with a good relationship between the cost of the procedure and the required time (Figure 4).

Conclusion

The Cervical Former is a matrix system that enables the dentist to make a high-quality Class V restoration in a short time. Compared to the “no-matrix technique” this kit provides the right professional tools for the realization of non-porous, smooth surface restorations that avoid incorporating an oxygen inhibition layer. Only very minor final corrections are necessary. The “compression” of the filling material to the walls of the preparation leads to better tightness between the filling and the dental tissues. Because of the matrix’s transparency, it is possible to control the filling material under the matrix.

The universality and simplicity are the most important features appreciated and they are unique for the Cervical Former. There is only one type of matrix for all clinical situations. The matrices can easily be trimmed to the required shape with usual scissors. The two ends of each matrix further save material.
