Inside Dental Hygiene (IDH): What are the most important considerations for dental hygienists in maintaining dental unit waterlines?

Amanda Hill, BSDH, RDH: The most important thing to do is to test your waterlines. Regardless of what protocol, products, and system you choose, every single product will fail at some point because biofilm is a beast. If you are not testing it, you have no idea if it is working. Beyond that, reading the instructions for use on any product is critical as well. All products work differently, and all products are not the same, so you cannot switch from one to another without shocking in between, because each one leaves behind a unique residue.

IDH: What about ultrasonic cleaners?

Hill: With ultrasonic cleaners, you need avoid any chlorine-, chloride-, or fluoride-based solutions, because they can affect the metal or even some of the resins that we are using now in the handles. Make sure that your cleaner is pH-balanced as well, and make sure that your ultrasonic is working; the fact that it is making noise does not mean it is cleaning and disinfecting your instruments.

IDH: What is most important in aerosol mitigation?

Hill: COVID made us so aware of aerosols. Using high-volume evacuation (HVE) for aerosol-generating procedures is very important, and we need to ensure that our vacuums are up to the task. If we are not maintaining our vacuums, the HVE system is not controlling any aerosols. Using a product that is pH-balanced is important here as well, so that it is safe for your amalgam separator. Make sure you are running your lines at least every day, preferably with some type of atomizer dispenser, which creates a vortex inside the lines to clean the inside walls and allow for better air flow.
