Historically the first-line antibiotic prescribed for penicillin-allergic patients with endodontic infections, clindamycin has been assigned a black box warning from the FDA as a result of reports of an increased likelihood for Clostridium difficile infection associated with its use. The purpose of this article is to discuss the risks of clindamycin outlined in recent studies and review the current literature-based prescribing guidelines in order to aid clinicians in making optimal treatment decisions for their patients.
